胆管癌(Cholangiocarcinoma,CCA)伴随着多种胆道系统癌症,现有治疗手段预后差。原发性硬化性胆管炎(primarysclerosingcholangitis,PSC)的发生是罹患胆管癌的危险因素。目前,由于缺乏可用的准确的非侵入性生物标志物,CCA与肝内胆管癌(intrahepaticCCA,iCCA)、肝细胞癌(hepatocellularcarcinoma,HCC)之间的非侵入性鉴别诊断非常困难。
为了寻找新的生物标志物,来自西班牙巴斯克大学的研究人员从CCA(n=43)、PSC(n=30)和HCC(n=29)患者和健康个体(对照组,n=32)中分离出血清的细胞外囊泡(EV),并检测其蛋白质含量。通过使用纳米粒子跟踪分析技术(NTA),研究人员发现HCC血清的EV浓度高于其他各组。圆形的形态(透射电子显微镜)、大小(NTA约为nm直径)和标记物(免疫印迹检测CD9、CD63和CD81)表明,被检测的大多数血清EV是外泌体。蛋白质组图谱(通过质谱)显示出多组差异表达的蛋白质。这些蛋白质中的几种显示出较高的诊断价值,CCA相比于对照组的ROC曲线下最大面积(AUC)为0.,CCAI-II期相较对照组为0.,PSC相较对照组为0.,PSC非肝硬化相较对照组为0.,CCA相较PSC为0.,CCAI-II期相较PSC为0.,HCC相较对照组为0.,iCCA相较HCC为0.。通过对人CCA细胞的EV的蛋白质组学进行分析,结果显示与正常人胆管细胞释放的EV相比,其中的致癌蛋白丰度更高。在免疫缺陷小鼠的肝脏中原位植入人CCA细胞,可检测到含有一些类似人致癌蛋白的EV在血清中释放。研究人员据此得出结论,CCA、PSC和HCC患者血清EV中发现了新的蛋白质组学特征,在诊断上具有潜在有效性。
